SERVICES PROVIDED:
Classes are taught by a professional and AKC certified dog trainer ("the Trainer") provided by Western States K-9 College ("WSK9CO"). The Trainer, at his/her discretion, may refuse entry to any dog which is not healthy, is aggressive or does not seem suitable for the services provided.
PAYMENT:
The Owner agrees to pay for all services at the time they are rendered. All charges must be settled by the Owner. In the event of a check being returned, for any reason, a state maximum fee will be assessed and cash payment may be required.
REMOVAL FROM TRAINING:
The Trainer, at his/her sole discretion, may remove a pet from class if a hazard or threat of any nature to any other animal or person is present. No refund will be given under such circumstances.
BEHAVIORAL/TRAINING ADVICE
All behavioral advice and training suggestions are provided at the Owner's own risk. Use of this information is voluntary and the Trainer is not responsible for consequences. Participation alone does not guarantee your dog will pass all tests required for certification.
PHOTOGRAPHIC RELEASE
The Trainer will occasionally use photographs and/or video of students and animals on web pages, during training sessions, in promotional materials and training videos. By your submission of this registration you agree to the usage of you and/or your dogs likeness in such material.
LIABILITY/HOLD HARMLESS:
I, ("the Owner"), as the legal owner of, below named dog, do hereby waive and release, WSK9CO and the Trainer from any and all liabilities of any nature. I agree to take complete responsibility for the actions of my dog, before, after and during class. At no time will WSK9CO or the Trainer be liable or responsible for the actions of myself, my dog or any other person who accompanies me to class. Owner agrees to indemnify and hold WSK9CO and the Trainer harmless from all liability for any loss, damage or injury to persons, animals or property arising from or related to Owner’s pet.
OWNER CERTIFICATION:
By typing in my name below and submitting this agreement, I am certifying that my pet is in good health and current on all vaccinations.
